Below, we outline how to obtain a permit in New York, New Jersey and Pennsylvania.<\/p>\n<h5>New York<\/h5>\n<h5>Who\u2019s eligible?<\/h5>\n<p>Applicants must live in New York to be eligible for a permanent parking permit but do not need to be a licensed driver or own a vehicle. They must also provide proof of a qualifying disability, defined as one or more of the following\u00a0impairments, disabilities or conditions that are both permanent in nature and affect mobility:<\/p>\n<ul>\n<li>use\u00a0of portable oxygen;<\/li>\n<li>legal blindness;<\/li>\n<li>limited\u00a0use, or no use, of one or both legs;<\/li>\n<li>inability\u00a0to walk 200 feet without stopping;<\/li>\n<li>a\u00a0neuromuscular dysfunction that severely limits mobility;<\/li>\n<li>a\u00a0Class III or IV cardiac condition (American Heart Association standards);<\/li>\n<li>severe\u00a0\u00a0limitation in the ability to walk due to an arthritic, neurological or orthopedic condition;<\/li>\n<li>restriction\u00a0because of lung disease to such an extent that forced (respiratory) expiratory volume for 1 second, when measured by spirometry, is less than 1 liter, or the arterial oxygen tension is less than 60 mm\/hg of room air at rest; or<\/li>\n<li>any\u00a0other physical or mental impairment not previously listed which constitutes an equal degree of disability, and imposes unusual hardship in the use of public transportation and prevents the person from getting around without great difficulty.<\/li>\n<\/ul>\n<p>A state facility or agency licensed by the state or locality may also receive a parking permit for vehicles\u00a0primarily used to transport people with qualifying disabilities.\u00a0<img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-8037 alignright\" src=\"https:\/\/www.smartsign.com/blog\/wp-content\/uploads\/2016\/11\/New-York-handicap-parking-permit-signs.jpg\" alt=\"new-york-handicap-parking-permit-signs\" width=\"141\" height=\"409\" \/><\/p>\n<div id=\"permit\">\n<h5>How to obtain a permit<\/h5>\n<\/div>\n<p>The New York Department of Motor Vehicles (DMV) does not issue handicapped parking permits. Contact the local issuing agent for specific information on how to obtain a permit.<\/p>\n<p>Regardless of the form, all applicants must present proof of disability, which can be shown in one of two ways:<\/p>\n<ol>\n<li>Have a medical doctor, osteopath, optometrist, or podiatrist\u00a0complete Part II (Medical Certification) of\u00a0<a href=\"https:\/\/images.myparkingpermit.com\/doc\/New%20York%20Form%20MV-664-1.pdf\">Form MV-664-1<\/a>. Podiatrists\u00a0must be licensed in New York; they can certify applicants only for severe disabilities of the foot;\u00a0or<\/li>\n<li>Obtain a statement from a doctor certifying the applicant\u2019s qualifying disability.\u00a0The statement must be less than a year old, describe the condition and how it limits mobility, be on the doctor\u2019s letterhead, and include the doctor\u2019s professional license number and signature. If the doctor practices outside of New York , the statement must identify his or her licensing state.<\/li>\n<\/ol>\n<p>A similar statement is also acceptable from<\/p>\n<ul>\n<li>\u00a0a hospital, clinic. or medical facility;<\/li>\n<li>the NYS Office of Vocational Rehabilitation in the NYS Department of Education; or<\/li>\n<li>the NYS Commission for the Blind and Visually Handicapped in the Office of Children and Family Services (for verification of legal blindness).<\/li>\n<\/ul>\n<p>Applicants with an\u00a0obvious, visible, permanent disability, such as the complete or partial loss of a leg or foot, may have the requirement for a medical statement waived by the local issuing agent.<\/p>\n<p>To apply for a temporary permit, applicants must present the same proof of disability as required for a permanent permit. The applicant must have\u00a0a driver license or other form of identification, though, and a\u00a0notarized statement or police report attesting to the loss or theft of the identification card and placard.<\/p>\n<div id=\"hang\">\n<h5>Where to hang the placard\/permit<\/h5>\n<\/div>\n<p>Placards must be\u00a0displayed on the rear view mirror of the vehicle whenever it\u2019s parked in a designated wheelchair\u00a0symbol parking space. Permanent placard holders will be mailed a renewal notice 60 days before the placard expiration date.
